Summary0004670: QFormBuilder unable to create extension custom widgets
Description

QFormBuilder relies on plugin paths being defined, which are currently set to
<slicer_install_folder>\bin\designer
<slicer_install_folder>\lib\QtPlugins\designer

Plugins from extensions are not installed to these folders, and so QFormBuilder cannot create them.

Steps To Reproduce

Call slicer.util.loadUI() with a .ui file that contains an extensions custom widget
