Tried loading different volumes on El Capitan and Sierra using Slicer 4.6. Cannot reproduce.

I removed the settings directory:
http://wiki.slicer.org/slicerWiki/index.php/Documentation/4.5/SlicerApplication/Settings

This solved the problem for me.
