Hi Ron,
I think the problem is caused by the new DWIConvert from BRAINSTolos and not the DICOM module. I invoked DWIConvert CLI with some sample DWI dicom files. Although it gives the message "DWIConverter completed without errors", it doesn't create any nrrd output. This is not the case with the old (before itk4) Dicom to Nrrd converter with the same inputs. The loading error you are getting is because there is no nrrd output available.

I checked Slicer mailing archive and bug tracker but there is no report on this issue. The latest reference I found was the thread at [1]. There is also no documentation page for DWIConvert. Should I report these issues to the bug tracker?

Regarding documentation, I fully agree that documentation is needed. This has been our our todo list for quite some time. As soon as other priorities subside we will prepare documentation. In the interim, we would greatly appreciate your contributions for documentation that you can provide :).

My guess is that we need to add a "fileExtensions=³.nrrd,.nhdr² to the outputVolume command line option so that slicer behaves properly. This will be a good task for David Welch to attack during the programmers week in Utah. I¹ll not have time to address this between now and then due to holiday and work obligations.

To reproduce the problem please use the AdultDWI_DICOM (Siemens mosaic) from data set [1]. DWIConverter gives the message "DWIConverter completed without errors" however it doesn't create any nrrd file at the output directory. The output volume node is also created but doesn't contain DWI information. The same settings in DICOM to NRRD Converter (Slicer 4.2) creates a nrrd output. I attached the CLI arguments for both tests. It would also be very helpful to have a documentation page for the new converter.
