Summary0002826: pagination in extension wizard is not appropriate
Description

When the extension manager comes up, the window should be sized to be about two thirds of the size of the slicer main window. The entire area available should be taken up with icons. This means that slicer should display by default the maximum number of icons that fit onto the window. There also should be some sorting capabilities.

The current layout of the extension manager window is very misleading: it seems as if the SlicerRT extension would not exist (see the attached MissingExtensions.png screenshot). Actually, the Reporting, SlicerRT, and TubeTK extensions are listed on the second page, but there is very little hint that a second page even exist.

Also, the template extensions should be removed from releases (shown only if a special "Developer" option is enabled in the Slicer configuration, or at least don't show them at the most prominent position in the default view).

The midas code has been updated to remove the need for pagination.

Note that the scroll bar still doesn't appear on Mac when showing the built-in extension manager. 0002333 is not resolved :(
