Summary0001375: QT_PLUGIN_PATH not set right on windows install
Description

os.environ['QT_PLUGIN_PATH']
'C:/Program Files (x86)/Slicer 4.0.gamma-2011-08-29/./lib/Slicer3'

Should have QtPlugins at the end.

Since the database driver is not loaded, ctk throws and exception and slicer crashes.

Steps To Reproduce

Install a recent slicer4 on windows.

Go to the DICOM module.

Set the dicom database to a valid directory.

Exit slicer and go back to DICOM module (crash).
